PILOT PLUS
Program Description:
Pilot Plus is a Housing and Urban Development Shelter Plus Care Program. Community Action Committee [CACCI] partners with the Barnstable Housing Authority and provides case management to two chronic homeless, disabled individuals. These individuals are selected from the participants of CACCI's Pilot House shelter. They are candidates who are considered to be ready for the next step of permanent housing. CACCI's Main Street Outreach worker provides in-kind case management services to these participants.
Goal: To provide shelter plus intensive case management services for two chronic, disabled, homeless individuals in order to empower and assist them in achieving their goals of residential stability, increased skills or income, and greater self-determination.
The Main Street Outreach worker develops with the participants mutually agreed upon service plans to achieve their goals. Participants agree to adhere to the Terms of Participation. The outreach worker monitors progress, assisting the individuals in accessing the benefits and services needed to support stability and their identified goals. Individuals work in compliance with the subsidy voucher requirements. Barnstable Housing Authority administers these subsidies. CACCI assists the participant in finding and maintaining a rental unit.
Milestones:
- Participants have maintained the same rental unit, some for over a year thus achieving residential stability and working towards greater self-determination
- Participants have gained and maintained employment, some for over a year
