TRANSITIONAL HOUSING PROGRAM

Program Description:

The Transitional Housing Program [THP] is a Department of Transitional Assistance [DTA] Emergency Assistance Shelter program for multi-needy families.

Goal: To provide shelter and intensive case management to multi-needy homeless families referred by DTA in order to empower and assist homeless families in becoming self-sufficient and housing ready. The ultimate goal for participants of the THP is safe, affordable permanent housing.

THP case managers assess potential families identifying their strengths and barriers to self-sufficiency and accessing permanent housing. Mutually agreed upon service plans are developed to address identified barriers and support families in achieving their goals. Families agree to adhere to the THP Terms of Participation. The case managers monitor progress in achieving the goals stated in the individualized service plans. Components of the service plans may include budgeting, education, employment, transportation, childcare, housing readiness and housing search, tenancy skills, counseling, safety plans, and substance abuse relapse prevention. Families work in compliance with DTA's Emergency Assistance Shelter regulations and Self-Sufficiency mandates. Families stay in the THP an average of 6-9 months.

Collaboratives: The THP partners with the Cape Cod Local Housing Authority Collaborative that provides up to 12 units of scattered site housing. Dennis Housing Authority is the lead entity of the Collaborative. The THP partners with local landlords who lease 13 scattered site units to Community Action Committee of Cape Cod and Islands.

Milestones:

  • The program works with on average 40 families annually.
  • 85% of participants have achieved targeted goals towards self-sufficiency
  • Over 50% of participants each year secure safe, affordable housing
  • Participants have been shown to achieve stability in the areas of substance abuse recovery, overall safety, paying bills and saving money, becoming employed and retaining jobs, and children school attendance and achievements
